RISE Housing and Support Services has an opportunity for an empathetic, organized, and dedicated individual to join our growing care management team. A successful candidate will be able work with the individuals on their caseload to develop goals and progress through those goals to aid in overcoming life challenges their mental health diagnoses have presented.
RISE offers a Care Manager:
- A starting rate of $22.23 per hour with a 36 hour workweek
- $1,500 bonus after 6 months of employment

Overview of Job Responsibilities:
- Developing and implementing care plans with program participants and supporting their efforts to attain their goals
- Complying with all relevant regulations governing the program
- Connecting program participants to community resources and housing options
- Modeling positive and appropriate behaviors and maintaining professional boundaries
To qualify you must have:
- A Bachelor’s Degree in a human services field
- Two years of experience in a related human services field
- An ability to operate independently, think on your feet, and follow your training
- Excellent organizational and time management skills
- Strong interpersonal boundaries
