Community Service Provider

RISE Housing and Support Services has an opportunity for an empathetic, organized, and dedicated individual to join our HCBS team as a Community Service Provider. A successful candidate will be able work with children and families to assist with advocacy, skills development, and local resource utilization.

RISE offers a Community Service Provider (CSP):

  • A starting rate of $22.23 per hour with a 36-hour workweek
  • $1500 bonus after 6 months of employment

Overview of Job Responsibilities:

  • Providing Home and Community Based Services (HCBS) per DOH and OMH guidelines
  • Meeting minimum caseload and service delivery requirements
  • Conducting assessments and developing person-centered strength-based plans of care
  • Documenting service delivery and ensuring chart compliance
  • Modeling positive and appropriate behaviors and maintaining professional boundaries

To qualify you must have:

  • A Bachelor’s Degree in a human services field
  • 2+ years experience providing direct services
  • Ability to operate independently, think on your feet, and follow your training
  • Excellent organizational and time management skills
  • Strong interpersonal boundaries
  • Our shared belief system in areas such as compassion, respect, positivity, and inclusion
  • Successful completion of RISE background checks, a valid driver’s license and a driving history acceptable to the RISE automobile policy
