RISE Housing and Support Services has an opportunity for an empathetic, dedicated, and compassionate individual to conduct targeted outreach and engagement individuals in the community actively using. A successful candidate will be able to meet individuals where they are at in the community to provide information on harm reduction, access to harm reduction supplies, and connection to available services.
RISE offers an OASAS OES Care Coordinator:
- A starting rate of $22.23 per hour with a 36 hour workweek
- $1500 bonus after 6 months of employment

Overview of Job Responsibilities:
- Provide person centered outreach and engagement to individuals actively using
- Create and foster supportive relationships with individuals who have substance use disorder (SUD)
- Actively connect individuals to recovery services and the recovery community
- Model positive and appropriate behaviors and maintaining professional boundaries
To qualify you must have:
- Any combination of 3+ years of education and experience with the target SUD population
- An ability to operate independently, think on your feet, and follow your training
- Excellent organizational and time management skills
- Strong interpersonal boundaries
- Our shared belief system in areas such as compassion, respect, positivity, and inclusion
- Clearance with all relevant background checks per RISE policies and governing regulations
- A valid NYS driver’s license and a driving record acceptable to the RISE transportation policy
- A willingness to travel throughout Saratoga county and some schedule flexibility (including some evenings and weekends)
