RISE Housing and Support Services has partnered with Corinth Central School District to provide Youth Care Management services with a physical presence in the town of Corinth. RISE Youth Care Management will have a satellite office in Corinth with fixed office hours and availability by appointment. A child and family can meet with a Care Manager at the satellite office, in the school, or at home.
“The goal of our district is to prepare and present a quality academic experience for all of our students and sometimes that means ensuring the availability of social services,” said School Superintendent Dr. Mark Stratton. “This partnership extends the services the School District offers students and families.”
Care Managers assist children and their families who struggle with chronic health conditions, mental and/or physical disability, complex trauma and/or behavioral issues. The Care Manager becomes the main point of contact in managing services for the child and family so that families do not spend countless hours trying to receive critical answers and information regarding services for their child, often including eligibility criteria.
“The Care Manager provides service coordination, ensuring each service provider works together and always in the best interest of the child and family,” explains RISE Executive Director Sybil Newell. “Care Managers are independent and advocate only for the child and family.”
Additionally, RISE is often able to provide immediate needs and referrals for respite, skill building and Family Support Services. RISE Youth Care Managers strive to learn about the unique needs of each family and work with them to make the most appropriate decision for their family.
“Youth Care Managers approach their work with the belief that every family has strengths, and that families are the natural and most effective drivers of their care,” explains RISE Care Management Program Supervisor Erika Straight. “We are committed to working with the family, strengthening all family members’ ability to navigate the maze of systems in order to access appropriate services, now and in the future. And with this partnership RISE Youth Care Managers will be working closely with the Corinth Central School District and their students and the community.”
