RISE Diversion Program, located in the Rain House community residence, is available to adults receiving services from the Saratoga County mental health system who need short-term, intensive 24-hour supervision.
The program provides shelter, supervision, and support to individuals in order to deter a crisis and/or avoid unnecessary hospitalization
We also provide step-down services for individual leaving a hospital setting.
Length of stay is dependent upon need, up to a maximum of 14 days. Extensions may be granted with administrative approval based on need.

- Guided practice in conflict resolution and stress management
Eligibility

- Be a resident of Saratoga County
- Residents must be at least 18 years old
- Have a primary diagnosis of a severe and persistent psychiatric condition
- Willing and able to actively participate in programming that will help them improve functional skills in order to move toward more independent living
The monthly fee for each resident is based on the current SSI rate for eligible people. Most residents pay this fee with income received from federal, state and/or local funding sources.
Referrals
Diversion Referrals are currently restricted to Saratoga County residents.
Mental health professionals may refer adults to Diversion submitting the Diversion Program Referral Form and a signed copy of the Diversion Program Rules. If you need help with this process or to learn more, please call for additional information.
