Supportive Housing provides permanent housing and support services to adults with mental health diagnoses and other life challenges.
Our specialized trauma-informed, person centered approach honors the unique needs and challenges of each person.
Residents work with staff to create individual goals and activities to assist them to live more independently.
We provide coaching, support, and guidance on an ongoing basis to enhance each resident’s ability to live successfully in the community.
Supported Housing Scattered Site Apartments: Residents are provided furnished single or double occupancy apartments throughout Saratoga County which includes a small number of apartments for families with children.
Riverview and Dominic Hollow ESSHI Apartments: Single occupancy supported housing apartments that are funded through the NYS Empire State Supportive Housing Initiative (ESSHI). Dedicated to chronically homeless persons that have mental health and/or substance use disorders, the ESSHI apartments are part of the New York State Governor’s comprehensive plan for affordable and supportive housing to ensure New Yorkers have access to safe and secure housing.
Dominic Hollow Apartments – located in Ballston Spa 18, unit are dedicated ESSHI apartments.
Riverview at Corinth EHSSHI Apartments – located in Corinth, 30 units are dedicated ESSHI apartments.

- 24-hour emergency on-call
- Furnished apartment
- Individualized support and interventions with behavioral health coaches
- Guided practice in setting and attaining short and long-term goals directed toward more independent living
- Ongoing review to evaluate growth and healing
- Advocacy and support in meeting both physical and mental health needs
- Trauma-informed, person centered independent living skills training
- Service coordination with community providers and natural supports
- Crisis intervention
- Guided practice in conflict resolution and stress management
- Social/recreational activities
Eligibility

- There are no residency requirements for eligibility but all our Supported Housing apartments are located in Saratoga County
- Residents must be at least 18 years old
- Have a primary diagnosis of a severe and persistent psychiatric condition
- Willing and able to actively participate in programming that will help them improve functional skills in order to move toward more independent living
Rental assistance is based on the resident’s income. Most residents pay their rent with income received from federal, state and/or local funding sources.
Referrals
Supported Housing apartments are located in Saratoga County, but we accept out of county referrals for Supported Housing.
Mental health professionals may refer adults to Supported Housing by completing the Saratoga County Adult SPOA Referral Form. If you need help with this process or to learn more, please call for additional information.
