Centrally located in Ballston Spa, RISE Above is a residential rehabilitation provides a structured environment for persons whose potential for independent living is seriously limited due to significant functional impairment.
This might include social, employment, or cognitive impairments, or the ability to follow social norms that requires restructuring social supports and behaviors in order to develop sufficient skills.
RISE Above provides a course of rehabilitative services in a structured environment with staffing to provide monitoring and support and case management.
Services Provided
Service provided are matched to the resident’s need for rehabilitative services including:
- 24-Hour Supervision
- Safe and supportive recovery environment
- Supportive services
- Structured activity and recreation
- Orientation to community services
- Overdose prevention education
- Individual, group and family counseling as appropriate to patient needs
- Group therapy
- Peer support
- Medical assessment of physical and mental health conditions
- Psychiatric assessment
- Medication management
- Psycho-social interventions
Eligibility & Fees
There are no residency requirements for eligibility.
An individual seeking residential rehabilitative services shall have an initial determination based upon face-to-face contact plus any other available records. If the initial determination indicates the person is appropriate for residential rehabilitative services, a level of care determination shall be made no later than 24 hours after the resident’s first on-site contact with the program.
Program fees are charged on the basis of program cost and a client’s ability to pay. Sources of payment include wages, unemployment insurance, pension, private insurance, private pay, or SSI/SSA.
Program staff will assist all clients to pursue avenues of financial support and clients are expected to meet program cost whenever possible.
Referrals
RISE Above is located in Saratoga County, but we accept out of county referrals. The program will consider self-referrals as well as referrals from all appropriate agencies, family members, and employers. Please call for more information.
